I founded True Care Transitions after recognizing a real gap in how families are supported when aging loved ones need to make significant life changes. During my years working in real estate, I often found myself assisting families as they sold a parent’s home, downsized, or transitioned into a senior living community. Time after time, I witnessed how overwhelming and emotional the process could be, so many moving parts, so many decisions to make, and rarely a single trusted guide to help coordinate it all.
That challenge became deeply personal when my own 90-year-old father needed to move into an independent living community. Suddenly, I was in my clients’ shoes. I spent weeks researching facilities, managing legal documentation like powers of attorney and living wills, arranging movers and estate sales, and coordinating the sale of his home so that everything would line up perfectly with his move-in date. I even found myself planning his new apartment, ensuring his favorite belongings could come with him and fit comfortably in his new space.
It was emotional, exhausting, and humbling. Despite my real estate experience, the process opened my eyes to how complex and taxing this transition can be for families, especially those without industry knowledge or a dedicated support team.
That experience inspired me to help build True Care Transitions. Our goal is to provide families with a single, trusted resource to handle every step of the journey—from legal and financial coordination to downsizing, moving, and settling into a new home.
At its heart, True Care Transitions was created to deliver compassionate, coordinated support that brings clarity, confidence, and peace of mind to families during one of life’s most sensitive transitions.
